Our readiness items, drawn from the published 6th edition and from what assessors are already asking for. It is not the criteria list itself: that runs to 92 criteria and lives in the Standards.
The 6th edition moves from three modules to four core standards and adds AI (F11), mandatory digital records (CG1.A), environmental sustainability (F3), antimicrobial stewardship (CG4.D), clinical risk management (CG7.A) and consumer engagement in place of the prescribed patient survey (PP8.A). The bigger shift is not the topics - it is that assessors are already looking for evidence across the whole three-year cycle rather than a folder built in the final quarter. Every item in this tracker is something we are implementing at our own practices now, so that the survey under the 5th edition is also a 6th edition rehearsal. Yes, on 26 August 2026. Accreditation is the separate question: the National General Practice Accreditation Scheme still assesses against the 5th edition, and the Australian Commission on Safety and Quality in Health Care has not published transition dates. So this tracker is for building readiness while you survey under the 5th.
